It’s a simple blood draw for a deep dive into your important health numbers. Blood draw requires fasting. Your screening results will not be shared with Local 71 or your Employer.

A Health & Lifestyle Survey that evaluates things like your emotional well-being, social support and other aspects that matter to your overall well-being.

Enrolled members and covered spouses will both receive a $75 prepaid Visa card by mail for completing the screening, lifestyle survey, and sending results to a physician. Members will also receive a $25 gas card for attending.

In addition to earning incentives, biometric screenings can help you better understand your overall health by identifying potential risk factors early, tracking important health indicators over time, and providing information you can use to have more meaningful conversations with your healthcare provider.

All participants will receive a free wellness program t-shirt, and members who complete the screening will be entered into our grand prize raffle.

Earn an additional $50 prepaid Visa card by completing other qualifying activities within the CHC website (fitness tracking, e-learning courses, etc.). A member and spouse have the opportunity to earn up to $275 in incentives while taking steps to improve their health.

While most incentives and benefits are tied to attending our Wellness Day on October 10, 2026, you can still complete your biometric screening through another provider if you're unable to attend. Alternative screenings must be completed by November 30, 2026. Instructions for completing an off-site screening can be found in the Onsite Lab & Physician Screening Guide.
