Important Notice Regarding 2016 Healthcare Coverage

What’s Changing Effective January 1, 2016?




  • We will be moving our medical coverage from Lifetime Benefit Solutions to Nova Healthcare (An Independent Health Company);

  • Provider Network – We will be using the Independent Health network

  • New ID cards with new ID numbers and Nova name and logo will be mailed to each member in December;

  • Nova card will now contain medical and dental information;

  •  Present the new ID card to your medical providers effective January 1, 2016;

  • Old LBS cards should be discarded after January 1st.



  •  Catamaran, our current Pharmacy Benefits Manager, is changing their name to OptumRx;

  • NEW Pharmacy ID numbers and OptumRx information will now be on a separate card and will be mailed to each member in December;

  • Present the new ID card to the pharmacy effective January 1, 2016;

  • Current LBS/Catamaran cards should be discarded after January 1st;

  • Nova Card will contain pharmacy information with the exception of your Optum ID.  Refer to Optum card for your new ID number.  Use subscriber’s (member) Social Security number as an alternate ID for all family members if you do not have your card.

  • Members Using Mail Order or Specialty - Optum will provide the new ID numbers to Mail Order and Specialty facilities to avoid disruption of service.  No action is required by the member.


What do I need to do to maintain current coverage?

  • Member Enrollment Update Form will arrive in the mail:

  • Review the information pre-populated on the form;

  • Make any necessary corrections to information;

  • Complete blank fields

  • Sign and date the back of the form

  • Return completed form to the Fund Office no later December 18, 2015;

  • Self addressed stamped envelope is enclosed for your convenience;


What is the Open Enrollment Period?

  • The Open Enrollment Period will run from November 1st – December 18, 2015;

  • The purpose of Open Enrollment is to enroll in healthcare coverage (if you are not currently enrolled) or to add eligible dependents or make any other necessary changes to your coverage;

  • Remember eligible dependents may only be added to your coverage during the Open Enrollment Period or during the plan year if you have a Qualifying Change in Status (marriage, birth, divorce, etc.).



  • Return Member Enrollment Update Form by December 18, 2015;

  • Watch your mailbox for new ID cards from Nova Healthcare and OptumRx and more information on the changes;

  • New ID cards will be a combination card for medical and dental and a separate card for prescriptions.  Show new card to medical and pharmacy providers effective January 1st.
